- Incident: An incident is a term used to record any deviation from a perfect attendance record.
- Perfect Attendance: Perfect attendance means having no absences or instances of tardiness during any twelve-month period.
- Absence: An absence is defined as an employee’s failure to report to work when scheduled. This applies to any required assignment, including a regular shift, overtime work, work-related meetings, or returning from lunch. Each day of absence will be considered one incident. A second day of absence will be considered a second incident, and so on. However, when an employee provides written documentation from a physician releasing them from work, the entire period of absence will be counted as one incident.
- Tardy: Tardiness occurs when an employee is not present and ready to begin working at their workstation at the scheduled start of their shift without prior approval.
- No Call/No Show: Employees must report their absence each day they are scheduled to work. Failure to do so will be considered a “no call/no show” and will result in immediate termination. Failure to report an absence at least 30 minutes before the start of a scheduled shift will also be regarded as a “no call/no show” and will count as one and one-half (1½) incidents.
Exceptions: Vacation, paid holidays, jury duty, paid funeral leave, job-related injuries, lack of work, military leave, and layoffs will not count as incidents. Pre-approved time-off requests and approved leaves of absence will also not count as incidents.
ANY EMPLOYEE WHO FAILS TO CALL IN AND/OR REPORT TO WORK FOR [NUMBER] CONSECUTIVE WORKDAYS WILL BE CONSIDERED TO HAVE VOLUNTARILY TERMINATED THEIR EMPLOYMENT.
REPORTING REQUIREMENTS
Employees must notify their supervisor at least 120 minutes, or two hours, before the start of their scheduled shift if they are going to be absent or late.
GUIDELINES FOR ATTENDANCE CONTROL
Based on the number of incidents occurring within a rolling three-month period, an employee will be subject to disciplinary action under the following guidelines:
- Two incidents within any 30-calendar-day period will result in a documented verbal warning.
- Three incidents within any 60-calendar-day period will result in a documented written warning.
- Any additional incidents will result in termination.
